Your TSH is normal. Your Free T4 is normal. You are still exhausted, cold, and gaining weight, and you have been told your thyroid is fine. Reverse T3 is one of the few tests that speaks to that gap.

What it tells you: how much of your thyroid hormone is being converted into the inactive form. Under stress, illness, chronic dieting or inflammation, the body can shunt T4 toward reverse T3 instead of the active T3 your cells use. Standard panels do not measure it.

Who orders it: people with persistent hypothyroid symptoms and normal standard labs, patients on thyroid medication who still do not feel right, and anyone whose functional or integrative provider requested it.
