Most of the testosterone and estrogen in your blood is bound to a protein called SHBG, and bound hormone cannot do anything. Only the free portion works. That is why two people with the same total testosterone can feel completely different.

What it tells you: how much of your sex hormone is actually available. High SHBG can leave you with symptoms of low testosterone despite a normal total. Low SHBG can do the opposite, and often travels with insulin resistance.

Who orders it: men whose total testosterone reads normal but who feel low, women working through PCOS or hormonal symptoms, anyone on hormone therapy tracking response, and people whose provider asked for it.
